Severe Chronic Sinusitis - Primary Ciliary Dyskinesia (PCD)
|
What it is PCD There are rare genetic disorders that predispose patients to sinusitis. Medical conditions that interfere with the body’s ability to remove any nasal pathogens or medical disorders that result in excess mucus production would be such conditions. Primary Ciliary Dyskinesia, (PCD) is also known as immotile ciliary syndrome and Kartagener’s Syndrome, is a rare genetic disorder that is a defect in the action of the cilia. The cilia are small hairs that line the sinuses, respiratory tract, and fallopian tubes. The cilia function to remove mucus and foreign pathogens from these passages in the body. The cilia are actually microscopic hairs that move in synchronized manner, and have a “beat” like cells in the heart. The cilia are known as the broom that sweeps out the mucus and other unwanted debris. When the cilia are not working properly, it results in sinus infections as well as many other very serious respiratory problems. Those with PCD need constant medical care, breathing devices and medications that can assist with removing excess phlegm to keep the airways unobstructed. The cilia actually are thought to beat 7 to 22 times per second and interference with this rhythm can result in excess mucus production or lack mucuciliary clearance. In addition, this lack of movement will also result in respiratory infection as well, with mucus and phlegm stuck in the respiratory airways. The impact from PCD is wide reaching. Possible medical conditions include:
Those with PCD or Kartagener’s Syndrome will possibly suffer with hearing loss due to the damage to the auditory internal structures. Loss of smell or a poor sense of smell is also a possible outcome as a result of the excess mucus production in the sinuses. PCD patients also experience the range of symptoms of post nasal drip, facial pain, headache, sleep disturbance, nasal congestion, nasal obstruction as well as loss of sense of smell. What is the best treatment for those with severe chronic sinusitis and PCD? Those with rare genetic disorders like Kartagener’s Syndrome or PCD have weakened immune systems with constant threats to the integrity of the system. While seriously ill on a regular basis they likely suffer with the body requiring more energy to respond to an attack on the immune system during a sinusitis infection. Any airway obstruction at night due to inflamed sinus passages, nasal congestion, sore-throat, and coughing disrupts sleep, resulting in less sleep during a time when your body needs sleep most. Chronic sinus infections last longer than 12 months, up to six months. Those with severe diseases may be chronically infected as those with a ciliary abnormality and will not be able to remove the mucus or the bacteria, fungus or viruses trapped in the sinus area. Those with chronic sinus infections are likely to have fever, as well as fatigue due to the constant presence of microorganisms in the sinus cavity. Complications from chronic sinus infections are many and can result in sinus surgery, nasal polyps, migraines, as well as more severe brain infection and in rare cases death. Physicians typically prescribe oral antibiotics with both wide spectrum and anaerobic antibiotics, to treat sinusitis. However, as medical advances continue and more research is presented less physicians are inclined to treat a sinus infection with oral medication. Due to recurrent sinusitis infections and lack of response to oral antibiotics, physicians are beginning to use a topical treatment for chronic sinus infections in lieu of the oral antibiotic. Sinus infection medications that are delivered intranasally are referred to as aerosol therapy which currently has a 98% success rate in treating the most severe cases of sinusitis. Aerosol therapy is successful in treating chronic and acute cases of sinusitis effectively with little to no side effects. This topical treatment is available through Sinus Dynamics. The best treatment for chronic sinusitis is a method that makes contact with the infection; a topical treatment delivers medication into the infected sinus area. Sinus Dynamics offers this topical sinus treatment with custom compounded: Antibiotics, Anti-fungals, Anti-inflammatories. With Sinus Dynamics:
